#BCCPA2018 Session: Community Cluster Model of Home Support

The healthcare system is often complicated for patients and providers alike. The growth in the seniors demographic in Canada will place an unprecedented demand on our healthcare systems—particularly on the continuing care sector.

Having capacity to meet this pending, and current demand requires creativity and flexible models of care. As a home support provider, Beacon Community Services is looking at new and creative ways to provide the care that B.C. seniors need in an efficient and effective way.

One example of this is the development of a cluster setting in high-density neighborhoods. Preliminary data obtained from surveying CHWs resulted in most respondents indicating an increase in job satisfaction following the implementation of the cluster model.

Join us as we share more in-depth examples of what is working well, increased staff satisfaction, client continuity, and the challenges we have faced in implementing this initiative.
