Integrating a Palliative Approach to Care by Having Conversations Early (IPACE)

In many instances, until an individual is in the midst of a health crisis or at the end of life, conversations about people’s wishes, values and goals are rarely initiated. Individuals may have received health care from different teams but are seldom engaged in discussions until a referral to palliative care services is initiated.

Because of the unpredictable nature of chronic disease, individuals living with frailty, dementia, or other non-malignancies, may not be connected to palliative care resources.

Holistic person-centered care should not be dependent on prognosis but responsive to people’s wishes, values and goals. All health care professionals need to have palliative care skills to sustain the needs of the aging population.

In this workshop, you will learn tools to easily identify individuals who may benefit from a palliative approach and enhance your communication skills to understand people’s wishes using the Serious Illness Conversation Guide. The outcomes of these conversations will play a valuable part in guiding your health care team to support and respect what matters most to individuals and their families.

Ingrid See, RN BSN, MEd, CHPCN (C)

Ingrid has worked extensively as a Clinical Nurse Specialist in the palliative care programs in home health and acute and residential care.  She started her home care nursing career in Boston, MA before returning to Canada to work in Richmond and Vancouver home health.   Ingrid completed her  Masters in Education with a focus on developing materials for cultural competency in palliative care.  Currently, Ingrid is the clinical lead for the Nancy Chan Palliative Care Ambulatory Clinic in Vancouver and the Integrating a Palliative Approach to Care by Having Conversations Early (IPACE) project

Sarah Lau,  RN BSN CHPCN (C)

Sarah has worked in Home and Community Health for over ten years and developed a passion early in her career for palliative when working with acute care palliative care teams. Though a decade ago, her professionalism and holistic centered care continues to inspire her practice when caring for people at home during their last stages of life. She finds it a huge privilege to walk with people along their journey and is passionate about palliative care. Outside of work you’ll find her training for half marathons, reading and travelling.

Assistive Technology to Improve Care and Quality of Life

CanAssist is a not-for-profit organization that operates out of the University of Victoria. CanAssist develops assistive technologies to individuals to improve their quality of life and to increase awareness of disability issues. In this workshop, CanAssist will showcase the Mobile Music Therapy Kit, the Ability411 website, and provide a general overview about creating customized assistive technologies.

The Music Therapy Kit is an all-in-one media kit that works hand-in-hand with CanAssist’s motion-activated music app, Waveband. The app and music kit were developed to allow individuals with disabilities the ability to express themselves musically without the need or know-how of traditional instruments.

CanAssist has recently launched the Ability411.ca website, a resource that provides information about commercially available technologies to seniors and their caregivers, in order to increase independence and well-being of seniors. Questions can be asked to Ability411 and customized responses are provided by CanAssist staff with oversight from the Canadian Association of Occupational Therapists.

Through community requests, CanAssist procures funding from philanthropic or other organization grants to develop customized assistive technologies for individuals. Regardless if the technology request is highly specialized for an individual or has the capacity to be broad-impact, CanAssist is committed to fostering inclusion and accessibility for everyone.
