#BCCPA2018 Session: Reducing ED Visits from Residential Care by Leveraging the Role of HCA

A 2014 CIHI report showed that a third of the Emergency Department (ED) visits from long-term care homes could have been avoided. Having a systematic approach for identifying residents at risk of health decline could help staff manage the resident within the residential care home and avoid unnecessary visits to the hospital.

This session will speak to PREVIEW-ED©, an innovative tool designed for Health Care Assistants (HCAs) that leverages their familiarity with residents and helps them detect subtle changes in resident health. The tool is completed daily by HCAs and uses simple language and uses a weighted scoring system to detect early health decline related to pneumonia, urinary tract infection, dehydration and congestive heart failure.

In 2016, a Fraser Health (FH) pilot in four residential care homes demonstrated a reduction of ED transfers for tool-sensitive conditions by 71%. This annualized projection based on the 17-week pilot motivated FH to implement the tool in all 79 of their homes in partnership with the Canadian Foundation for Healthcare Improvement (CFHI). Evaluation results showed several success factors, and lessons learned. Benefits included greater involvement of HCAs in decision-making as valued and respected members of the care team, and enhanced communication amongst and between groups of healthcare providers. Data has shown an impact on ED transfers for residential care homes that had been consistent in the application and spread of the tool.
