#BCCPA2018 Session: The Value of Safety Improvement Collaborative for Home Care Providers: Strategies & Outcomes

Home care providers are constantly challenged to improve the quality of home care and ensure patient safety. However, this is not always an easy task due to competing priorities and limited resources. Attend this presentation to learn how a small, emerging home care provider discovered a method to shift the bar by participating in wave two of The Home Care Safety Improvement Collaborative, a successful initiative supported by the Canadian Home Care Association (CHCA) and the Canadian Patient Safety Institute (CPSI).

This presentation zeroes-in on the topic of ‘Advance Care Planning and Serious Illness Conversations’ and explores how home care providers can help clients make informed decisions about current and future medical and personal care.

Presenters will demonstrate how they were guided by industry experts to successfully shift the number of clients with Advance Care Plans from 18% to 60%. Presenters will also share how project participation helped them apply a quality improvement methodology to measure, evaluate and track success, consult with clients and caregivers in improvement initiatives, and build high functioning teams.

This engaging presentation will highlight invaluable best practices, lessons learned, and quality improvement tactics that can be applied by agencies of all sizes.
